Welcome to our practice. We appreciate the opportunity to serve your health care needs. The following information is intended to provide a brief overview of our practice’s policies and procedures from scheduling your appointment to billing. To aid us in providing you complete services and for your safety, please bring all your current medications including prescribed and over the counter medications. You may also contact our office staff by phone at (281) 493- 3681 from 8:00 a.m. to 4:30 p.m. and we will be happy to answer your questions.

Appointments/Cancellations
To schedule an appointment please call at 281 493 3681. We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, please inform us 24-hours in advance. Failure to do so will result in a no-show fee of $25.00 We offer convenient appointment reminders via email and calls. Same day appointments cannot be cancelled without this no-show fee.
After hours Calls
A 24-hour coverage is available to all patients.If you have a real medical emergency please call 911 and/or visit an emergency room. That facility will contact our office as necessary and keep us involved. If you need to speak to the doctor after office hours, call office at 281 493 3681 and the answering service will take your message and contact the physician on call. Please provide the answering service with your name, date of birth and your pharmacy and a telephone number where you can be reached. Please do not call the answering service for appointments, lab results, or medication refills. These will be handled during regular business hours by our office staff.
Prescription Refills
If you need your prescriptions refilled please call your pharmacy first. If your pharmacy does not have an existing refill, please call the office and provide your full name, date of birth, name of the medication(s), dose, and pharmacy name and phone number. The staff will let you know if medications will be refilled or you will need to schedule an office visit. Please give at least 2 business days for refill approvals. Requests made after 2:00 PM will be handled the next business day.
Emergencies
In case of true emergency call 911 or proceed to the nearest hospital emergency room where the ER physician on duty will evaluate, treat and notify your physician
Payment
Deductibles, co-pays and other payments are expected to be paid at the time of service. You may pay by cash, check or the following credit cards: American Express, Visa, MasterCard and Discover. Statements are sent to your patient portal as and when necessary.
